Toronto Bridge and All-on-Four
The Toronto Bridge is a fixed prosthesis screwed onto dental implants, capable of replacing an entire dental arch. It is secured using the All-on-4 technique, which involves four implants and can replace up to 10-12 teeth. For even greater stability, six implants (All-on-6) can be used.
The Toronto Bridge was designed and developed by a group of Swedish researchers led by Prof. Branemark and was presented to the international scientific community at the World Dental Congress in 1982 in Toronto.
How the Toronto Bridge is Applied
First, the implants are inserted, followed by the placement of a temporary prosthesis. The final phase involves constructing the definitive prosthesis. The best results are achieved by waiting three months for the lower arch and six months for the upper arch. The definitive prosthesis is created after taking a precision impression: a metal or fibreglass framework is constructed, onto which the final resin teeth are modelled.
The Toronto Bridge features a resin flange (or artificial gum). This serves two main purposes: compensating for bone loss and covering the implants, which can be placed in the most suitable positions without aesthetic concerns.
The flange does not pose aesthetic issues as it is positioned beyond the smile line, making the transition between artificial and natural gum invisible. However, meticulous hygiene is required under the flange, where food debris can easily accumulate.

If immediate loading is not preferred, a temporary prosthesis is placed immediately after surgery, followed by the definitive one after osseointegration.
