Bone Reconstruction in the Upper Jaw Before Dental Implants

When a tooth is lost, the bone that once supported it may gradually lose volume. This can become important when a dental implant is planned, because an implant needs a sufficiently stable bone foundation.

A Sinus Lift can help, in selected cases, to rebuild missing bone volume in the upper jaw. The aim is to create better conditions for carefully planned implant treatment. Whether this procedure is appropriate depends on the clinical findings, diagnostic imaging and the patient’s general health situation.

What Is a Sinus Lift?

A Sinus Lift is a surgical bone reconstruction procedure in the upper jaw. It is usually performed in the area of the molars and premolars, where the bone lies close to the maxillary sinus. If the remaining bone is insufficient, placing an implant without bone reconstruction may not be advisable.

During a sinus lift, the floor of the sinus is carefully elevated. A suitable bone grafting material is then placed into the created space. Over time, this area can become a more stable foundation for a future or simultaneous dental implant.

In many cases, the sinus lift can be performed at the same time as implant placement. In other cases, the bone reconstruction is performed first, and the implant is placed only after a healing period. This timing depends on the individual clinical situation.

What Is the Maxillary Sinus?

The Maxillary Sinus is one of the paranasal sinuses. It is located in the upper jaw, above the back teeth. It is lined with mucosa and communicates with the nasal cavity through small openings.

For patients considering implants, the important point is simple: after tooth loss in the back of the upper jaw, the available bone height may be reduced. Before implant treatment, it is therefore important to assess how much bone is present and whether bone reconstruction may be needed.

How the Procedure Is Planned and Performed

Planning begins with a careful clinical examination and diagnostic imaging. A digital panoramic X-ray may be used as a first step. In many cases, 3D Cone Beam Imaging can provide more detailed information about bone height, bone width and the position of the sinus.

At FreeSmile Dental Clinic in Rome-Prati, 3D diagnostics are used when they are medically useful for planning. They help us understand the patient’s individual anatomy more clearly and prepare the treatment with greater precision. Technical statements about radiation exposure and image resolution should be checked internally against the device specifications before publication.

The procedure is usually performed under local anaesthesia. After the gum is opened near the sinus area, the sinus membrane is carefully lifted. A  biocompatible bone grafting material, such as Bio-Oss®, is then placed. The area is closed with sutures.

Some swelling may occur in the days after surgery. Healing is monitored over time. As the area remodels, it can contribute to a more stable foundation for a planned or simultaneous implant treatment.

Safety and Clinical Assessment

A sinus lift is a common procedure in implant dentistry today. Still, it remains a surgical intervention and should always be based on a careful diagnosis.

Before treatment, we assess not only the amount of bone available but also factors that may influence healing. These include smoking, uncontrolled diabetes, osteoporosis, certain medications and oral hygiene.

A good decision does not begin with the question: “Can an implant be placed?”
It begins with a better question: What conditions are needed for the treatment to be planned responsibly?

Oral Hygiene and Long-Term Maintenance

Bone reconstruction and implant treatment do not end with surgery. Long-term stability also depends on oral hygiene, regular check-ups and careful maintenance.

Important steps include:

  • Cleaning teeth and implant areas carefully at least twice a day.
  • Use dental floss or interdental brushes as instructed.
  • Attending regular dental check-ups and professional hygiene visits.
  • Following the recommended maintenance intervals, often every 4 to 6 months.

This helps turn a surgical procedure into a structured long-term treatment pathway.
