An infection around the Tooth Root can cause pain, swelling or discomfort. Sometimes it remains silent and is only visible on an X-ray. When a Root Infection persists after previous treatment, the important question is: Can the tooth still be preserved, or is removal the more reasonable option?
An Apicoectomy, also called Root-End Surgery, may help preserve the tooth in selected cases. The decision depends on the Diagnosis, the X-ray findings, the condition of the tooth and the expected long-term stability.
What is an Apicoectomy?
An Apicoectomy is a minor surgical procedure performed at the end of the Tooth Root. The infected Root Tip is removed, and the surrounding area is cleaned and disinfected.
This procedure may be considered when an infection near the Root Tip cannot be resolved through Root Canal Treatment alone. In some cases, the end of the canal may also need to be sealed from the root end.
The goal is not simply to perform surgery. The goal is to understand whether the tooth can be preserved in a reasonable and stable way.
When can an Apicoectomy be useful?
An Apicoectomy may be considered when an infection at the Root Tip persists or returns after previous Endodontic Treatment.
Common situations include:
- Dental Abscess: A collection of pus caused by a bacterial infection, which may extend toward the Root Tip.
- Periapical Granuloma: Inflammatory tissue around the Root Tip, often linked to a persistent infection.
- Persistent Symptoms after Root Canal Treatment: If pain, pressure or radiological signs of inflammation remain, the cause should be assessed carefully.
In these situations, removing the infected Root Tip may help control the infection and protect the surrounding bone. Whether the procedure is appropriate always depends on the individual Diagnosis.
How is an Apicoectomy performed?
The procedure is usually performed on an outpatient basis under Local Anaesthesia. The duration depends on the tooth position, the root anatomy and the complexity of the case.
Main Steps of the Procedure
- Careful Diagnosis Before surgery, the tooth, the Root Tip and the surrounding tissue are assessed. X-rays help identify the exact location of the inflammation.
- Local Anaesthesia The area is numbed so that the procedure can be carried out in a controlled and careful way.
- Access to the Root Tip A small opening is made in the gum to reach the affected Root Tip.
- Removal of the Root Tip The inflamed tissue and the end of the root are removed.
- Cleaning and Disinfection The surrounding area is cleaned. Depending on the Diagnosis, the canal may also be sealed from the root end.
- Sutures and Follow-Up The area is closed with sutures. The patient receives clear instructions for healing, wound care and follow-up appointments.
